NCOA: 10 Recommendations to Promote Healthy Aging
The National Council on Aging has offered 10 recommendations to promote healthy aging.
Enhance access to family caregiver support programs for veterans and federal employees and chronic disease self-management education (CDSME), which has been proven to improve health and functional outcomes and reduce hospitalizations and emergency room visits.
Expand the patient-centered medical home self-management quality standards to other delivery models for Medicare.
Strengthen the annual Medicare wellness visit to promote healthy aging to better address the needs of older adults with multiple chronic conditions.
Provide assistance to states on how to incorporate evidence-based healthy aging programs within their Medicaid programs.
The Center for Medicare and Medicaid Innovation should develop and test integrated self-care planning where primary care and community services providers integrate support to held older adults and caregivers reach personal goals for aging well.
Add second falls as a hospital readmissions reduction program measure.
Fund a Medicare demonstration modeled after the Medicaid incentives for Prevention of Chronic Diseases program.
Include CDSME in new Medicare billing codes for complex chronic care.
Provide funding to develop sustainable models for integrating community-based organizations offering CDSME with health care
providers to improve health care outcomes and save health care dollars.
Increase prevention and public health fund allocations for evidence-based CSME and falls prevention programs.
